For your first visit, we need some information about you and your insurance. You can complete the forms online. Here are links to complete the new forms online. 

  • You may also download the PDF forms and either email them to our staff or bring them to the office prior to your appointment.

  • You may call the office 805.983.0717 and they can take the information over the phone.

  • Or, you may complete the forms during your first visit. Please allow 15-30 minutes prior to your appointment.

Membership Plan

Carson and Acasio offers membership plans with affordable patient options for those who don't have insurance. There are three plans tailored for children and adults and start at $29 per month, with a 5% discount for each subscription for additional family members who are added to your membership.

In addition, patients enrolled in one of the plans receive a 20% discount for other dental treatments not covered under the chosen plan.

You may access and complete their application here. Once approved, the Care Credit card may be used at other medical practitioners which accept the card.

We offer several convenient ways for you to pay for your dental treatments.

 

>> Insurance. We accept most PPO dental insurance plans. We also have our own in-house dental insurance program. Since all insurance companies compute benefits differently, it is important for you to contact our insurance coordinator to determine how much your company will pay, and how much you will be responsible for paying yourself. 805.983.0717

>> We accept all major credit cards, debit cards, checks and cash.

Appointments Policy

To make an appointment, please complete the Request for Appointment form or call our office 805.983.0717. Emergency appointments are available 24/7.

We make every attempt to schedule appointments for our patients in a manner that reduces any waiting time and provide prompt and attentive service to each and every patient. We do not double book appointments and make every effort to be ready for you at your scheduled appointment time. We expect our patients to respect their scheduled appointment times and make every effort to be as on time for us as we are for you.
 

We require 48-hour notice for any appointment change. Failure to do so could result in a broken appointment charge. A broken appointment is a loss to you and prevents us from providing you with needed preventive and restorative care. It is a loss to the patient who could have had that appointment time, and it is a loss to our team who was fully prepared for your visit. Keeping your scheduled appointments and being on time is an important part of what contributes to our team providing the care our patients are accustomed to. We realize changes may need to be made 
occasionally, but we respectfully ask for your attention to this matter. Longer surgical and specialty procedures may require an initial payment to reserve appointment times.
